Therapeutic Injections
Campus Health provides a convenient location for the continuation of your injectable therapies while you attend NC State University. If you get regular therapeutic medication injections from a medical provider, under most circumstances, you can transfer your therapeutic medication injections to Campus Health to ensure that your current treatment plan is continued. Your prescribing medical provider will continue to manage your care and Campus Health will serve as a temporary extension of your provider’s office while you are attending NC State University. You must provide documentation (outside records) from your current prescribing provider prior to scheduling an initial appointment. This initial appointment will establish care with a Campus Health provider.
Prior to Your Initial Visit
- Complete your Health History Form (found on the HealthyPack Portal).
- Complete the Authorization to Release Protected Health Information Form allowing our office to communicate with your current medical provider.
- Upload the following documentation through the HealthyPack Portal or your provider may fax documents to 888-972-4158.
- The documentation must include ALL of the following information:
- Your diagnosis
- The name of the medication, the strength/dose, frequency, and how it is given
- Directions pertaining to an early and/or late schedule, if applicable
- Recommendations for Epi-Pen, if applicable
- When medication was last administered
- Other relevant patient history regarding your diagnosis and/or treatment
- Outside Physician information (name and contact information)
- The documentation must include ALL of the following information:
- After you have submitted all of the required information, you can schedule an initial appointment with the Campus Health provider through the HealthyPack Portal or by calling 919-515-2563.
- The appointment type should be “Chronic Condition Initial” if you are scheduling through the HealthyPack Portal.
- Once you have seen the Campus Health provider and they have reviewed all of your information, you will be notified that you can schedule your first injection appointment.
- Please note that your injection may not occur on the same date as the initial appointment.
What To Bring To Your Initial Injection Visit
- Your medication, clearly labeled with your name and date of birth
- You are responsible for storing the medication as directed by the manufacturer and removing it from refrigeration if applicable. Campus Health does not store any medications for patients.
- If you need additional storage space for your medication, you will need to communicate with University Housing and the University Disability Resources Office.
- Your Epi-Pen (if required) — make sure it is not expired
Fees for Injections
There are fees associated with your injections. Prior to your first appointment, please contact your health insurance provider to discuss your coverage and fees. It is the student’s responsibility to confirm that Campus Health has their current insurance information on file. Charges will be submitted to insurance, and any charges not covered by insurance are the responsibility of the student.
