Patient Rights

As a patient at NC State University Student Health Services you have the RIGHT to:

Receive Respectful Care and Treatment.

You will be treated with appropriate dignity and consideration by all staff members.

Participate in Your Health Care and Treatment.

You are a full partner in your own health care and will participate in decisions about your care and treatment.

Know Your Health Care Providers.

Health professionals in the Student Health Services are professionally trained and licensed in their field by the State of North Carolina. Specific information about our staff’s training, credentials and experiences is also available to you.

Confidentiality.

The specific information about your medical history and current health status is referred to as Protected Health Information (PHI). PHI contained in your medical record will not be released to any unauthorized individual without your express consent or a court order.

Accurate Information.

You will be given, to the extent known, information concerning your diagnosis, treatment and the prognosis for illness, injury or other health-related condition(s). This includes appropriate alternatives to care at Student Health Services when requested.

Fee Information.

You shall have access to information about fees for services involving additional charges not covered by the Student Health fee. Fees are posted on our billing page.

A Referral for a Second Opinion.

You will receive an appropriate response to any request for a second opinion regarding diagnosis or treatment. Note: Referrals made to providers outside Student Health (for a second opinion) are not covered by the Student Health fee.

Be Informed of Advanced Directives

You have the right to information about what an advance directive is and how to create an advanced directive is also available on our website.

Request to change provider.

You have the right to request to change your health care provider if another qualified health care provider is available. Note: Any request for change in health care provider may result in a change in the day and time of your scheduled appointment.

Patient Responsibilities

As a patient at NC State University Student Health Services you have the RESPONSIBILITY to:

Be Candid with your Health Care Provider.

It is important to provide full, accurate information about your illness or problem, past illnesses, and medications to allow proper evaluation and treatment. Let your provider know if your condition worsens or your recuperation does not follow the expected course of recovery.

Ask Questions. Take Notes.

Ask specific questions to ensure you understand your health problem, as well as the health care provider’s recommendation for continuing care and any costs associated with that care. Take notes so that you don’t forget instructions. Studies show that if notes are taken patients recall what they were instructed to do.

Voice Your Concerns.

The Student Health Service team will be better prepared to improve upon service delivery if they are informed of concerns about the care you receive or interactions with staff. Ask to speak with a manager or member of the senior leadership team. Your comments and ideas can be submitted in person, on line or in writing, and are both welcomed and appreciated. Our Director welcomes conversations with patients and parents and can be contacted at 919-513-3285 during business hours.

Treat Others Courteously.

Student Health Services staff members and other patients are people like yourself, trying to do the best job they can. Please treat them with the courtesy and respect you expect them to show you. Be on time for your appointment. Plan to arrive early to secure parking and check in. It is also helpful to our staff and other patients when you let the Student Health Services staff know as soon as possible if you cannot keep an appointment.

Help Protect Your Own Confidentiality.

Do not lend your student identification card to others. It may lead to abuse, inaccuracies in your medical record or student billing account or temporary interruption of your privileges to use student health until the misuse can be resolved.

Follow Your Treatment Plan and Use Medications Properly.

Do not give, or sell, medications prescribed for your needs to others. There may be a complication, such as adverse medication interactions, which could cause harm to another person.

Transportation.

It is the responsibility of SHS patient to arrange accompanied transportation if indicated by provider.
