Confidentiality.

The specific information about your medical history and current health status is referred to as Protected Health Information (PHI). PHI contained in your medical record will not be released to any unauthorized individual without your express consent or court order.

Patient Responsibilities

As a patient of NC State University Campus Health, you have the responsibility to:

Be candid with your health care provider.

It is important to provide full, accurate information about your illness or problem, past illnesses, and medications to allow proper evaluation and treatment. Let your provider know if your condition worsens or your recuperation does not follow the expected course of recovery.

Ask questions and take notes.

Ask specific questions to ensure you understand your health issue, as well as the health care provider’s recommendation for continuing care and any costs associated with that care. Take notes so that you don’t forget instructions. Studies show that if notes are taken patients recall what they were instructed to do.

Voice your concerns.

The Campus Health team will be better prepared to improve upon service delivery if they are informed of concerns about the care you receive or interactions with staff. Ask to speak with a manager or member of the senior leadership team. Your comments and ideas can be submitted in person, online or in writing, and are both welcomed and appreciated. Our director welcomes conversations with patients and parents, and can be contacted at 919-513-3285 during business hours.

Treat others courteously.

Campus Health staff members and other patients are people like yourself, trying to do the best job they can. Please treat them with the courtesy and respect you expect them to show you. Be on time for your appointment. Plan to arrive early to secure parking and check-in. It is also helpful to our staff and other patients when you let the Campus Health staff know as soon as possible if you cannot keep an appointment.

Help protect your own confidentiality.

Do not lend your student identification card to others. It may lead to abuse, inaccuracies in your medical record or student billing account, or temporary interruption of your privileges to use Campus Health until the misuse can be resolved.

Follow your treatment plan and use medications properly.

Do not give or sell medications prescribed for your needs to others. There may be a complication, such as adverse medication interactions, which could cause harm to another person.

Transportation.

It is the responsibility of Campus Health patients to arrange accompanied transportation if indicated by the provider.

NC State University Campus Health does not discriminate due to race, color, creed, national origin, religion, age, sex, disability, veteran status, or sexual orientation. Campus Health welcomes all eligible students and the campus community to the center and will provide care and deliver services in a non-discriminatory manner.
